Passport Funding Supports:
The Passport program is a reimbursement program that helps adults with a developmental disability participate in their community. You can either choose to receive the money directly, or purchase your supports and services through an agency. Passport provides funding for services and supports so you .
There are three way that you could access your passport funds. Strong-Together is a convination of Broker Agency and Service Agency

Someone such as a family member or caregiver who directly manages the funding on behalf of a Recipient.

A person who is being supported by the funding.
Broker or Service Agency
Broker or Service Agency
A Service Agency or Broker is an entity that manages the funding on behalf of Recipients that is not a caregiver.

Our organization will primarily provide assistance and understanding of how to access your passport funds.
Passport Fund Planning and Budget monitoring
Ensure that purchases align with the goals and principles of the Passport program
Assistance in accessing services and activities
Submit claims on a regular basis (monthly) with all required supporting documentation
If you hire Strong-Together as a broker, our organization will manage your passport funds directly and will provide the resources to access the services and activities.
The recipient will have to sign an agreement that will allow Strong-Together Inc. to access your passport account until the DSO (Payee and Notification form).
