1. Executive Summary

This agency will provide support services for people with developmental disabilities to help them live meaningful, independent, and inclusive lives in their communities. The organization will offer programs that focus on life skills, communication, leisure activities, literacy, and community participation.

Our goal is to create a safe, respectful, and empowering environment where individuals can develop their abilities, build relationships, and achieve personal goals. Through trained staff and person-centred planning, we will provide high-quality support that improves quality of life for individuals and their families.

The agency will operate in Ontario and work with community partners, families, and social service organizations to provide inclusive and accessible programs.

Regardless of the service required, our organization will develop an Individual Support Plan to identify the objectives and goals. Services like leisure activities will require a less detailed ISP, but also it will be assessed according to the frequency requested. The cost of the service also will be assessed according to the ISP and passport funds.
